Free Mobile Pediatric Asthma Clinic May 5th & 12th

Does playing and/ or exercise leave your child short of breath? Does your child cough at night? Does your child use their rescue inhale more than once a week?

Parents:  If you can answer “Yes” to any of these questions above or just concerned about your child’s breathing.  We can help by preforming a free pulmonary function test and an assessment done by a Nurse Practitioner.  If needed an Asthma Care Plan will be written for the school nurse.  All will be performed at no cost to you or your insurance.   If you would like an appointment for a FREE SCREENING please fill out the form that was sent home with your child and return to your school nurse or call Laurie Smith RN at 903-856-6482. (You will receive a call to set up appointment) 

We will be at your child’s school on: May 5th and May 12^th

** Parent or Guardian must accompany child to appointment ** Please bring all medication your child is taking to appointment
